The following are common forms that students may need while arranging for Honors credit. These forms are also available to pick up in Hillcrest 137, and may be dropped off there as well.

• 4000/5000-Level Course Approval Form
• Faculty-Student Agreement (to be completed at the beginning of the semester)
• Faculty-Student Agreement Approval Form (to be completed at the end of the semester)
• Graduate-Level Course Approval Form
• Independent Study/Research/Tutorial Approval Form
• Thesis Approval Form
